You can renew your passport if:

• you are able to submit your most recent US passport, or;
• your most recent passport was issued in your current name, or;
• your most recent passport was issued within the past 15 years, or;
• your most recent passport was issued when you were 16 years of age or older.

Required Documents

1. Your most recent passport.
2. Two passport photos.
3. Complete the online Passport Application By Mail, form DS-82.
   • After completing the form, you must click the "Create Form" button at the bottom to generate a printable PDF version.
   • Print two copies. Photocopies are not acceptable.
4. If your name has changed since your passport was issued, enclose a certified copy of the legal document specifying your name change (e.g. marriage certificate, divorce decree, adoption decree, or court order). No photocopies accepted.
