• You are applying for a U.S. passport for the first time
• Your previous U.S. passport was lost, stolen, or damaged
• Your previous U.S. passport has expired & was issued more than 15 years ago
• Your previous passport has expired and it was issued when you were under 16.
• Your name is changed since your passport was issued and you do not have a legal document formally changing your name.

What You Need

• Application for Passport,Form DS-11, completed Forms can be obtained from any passport agency or acceptance facility. NOTE: Please do NOT sign the DS-11 application form until the Passport Acceptance Agent instructs you to do so.
• Birth certificate issued by the city, county or state or US Naturalization certificate
• Valid current driver's license
• Two Passport Photos
• Power of Attorney Authorization, completed and signed

Service Fees

1. Pay the passport processing fee at the Post Office. They will inform you what that is.
2. Include a check for the applicable fees:
   • Passport processing fee. Pay at the Post Office.
   • Service Fee:
     • $ 90.00 - 5 business days
     • $130.00 - 3-4 business days

What to Expect

Within 5 business days of receipt, we will hand-deliver your passport application to the Federal Building in Los Angeles. Once your passport has been issued, we will return your application materials using the return shipping you specified.
