Lost or Stolen Passports

If you had lost your passport in the United States, there are two steps required to get a new passport. First, you must report your valid passport lost or stolen immediately. This is required of anyone who loses or has a passport stolen. The second step is to replace your passport. Both steps and forms required are described below.

Required Documents

  • The passport number of your lost passport.
  • Passport Application, form DS-64 regarding a lost or stolen passport.
  • Two passport photos.
  • Passport Application By Mail, form DS-82.
    • After completing the form, you must click the "Create Form" button at the bottom to generate a printable PDF version.
    • Print two copies. Photocopies are not acceptable

What to Do

  1. Bring the required documents to any U.S. Post Office with passport services. They will help you assemble the documents in a sealed envelope. Don't forget the authorization letter for the power of attorney!
  2. Pay the passport processing fee at the Post Office. They will inform you what that is.
  3. Include a check for the applicable fees:
    • Passport processing fee. Pay at the Post Office.
    • $ 90.00 USD - 5 business days
    • $130.00 USD - 3-4 business days

What to Expect

Within 5 business days of receipt, we will hand-deliver your passport application to the Federal Building in Los Angeles. Once your passport has been issued, we will return your application materials using the return shipping you specified.

  1. Get your paperwork together. Please fill all forms carefully. We are not responsible for any delays if your information is inaccurate. Check the requirements for what forms and copies you need to make. Please contact us if you have any questions.
  2. Include payment You'll need to calculate any government processing fees, service charges, and return shipping. Make checks for service fees and return shipping payable to "US China Professional Visa Center.
  3. Send it in! Send the sealed envelope to:

We charge a flat service fee for each service provided. Shipping and handling charges are based on the return shipping option you select. All amounts are in United States Dollars
$ 30.00 FedEx Letter (Next Business Day - Up to three passports) - Continental USA Only
$ 18.00 FedEx Express Saver 3rd business day - Continental USA Only
$ 45.00 FedEx Letter (Saturday Delivery) - Continental USA Only
$ 58.00 FedEx Next Day Letter (No Saturday Delivery) - Alaska, Puerto Rico, Hawaii, US Virgin Islands only
